Job Summary:
Provides continuing, comprehensive health maintenance and medical care to entire family regardless of sex, age or type of problem.

Qualifications:
1. Successful completion of a Doctor of Medicine degree or doctor of osteopathy degree from an approved school of medicine or osteopathy.
2. Board Certified or Board Eligible required
3. License with AZ Medical Board.
4. Active DEA
